The consultation gives clients a structured place to organize what is happening, understand available pathways, and prepare for conversations with their care team.

A review of your current care situation and main concerns
Assistance preparing questions for doctors, nurses, specialists, palliative care teams, hospice teams, or other providers
Advance Care Planning support, including values, priorities, and Substitute Decision-Maker preparation
Help identifying practical next steps
Relevant resources for care planning, caregiver support, grief support, hospice, palliative care, or community services
Access to The Crossing Room
A private, moderated message board gives families and supporters a safe, invitation-only space to share updates, memories, and practical information without the noise of social media. Every post is gently overseen to keep the environment calm, respectful, and focused on connection.
What This Service Does Not Provide
This service provides care planning, health-system navigation, general education, and preparation support. It does not replace medical, legal, financial, counselling, emergency, or crisis services.

How to Book
Choose Your Date
Use the booking link to view the live calendar and select an available date that works for you.
Submit Your Request
Complete the booking form and submit payment. Your booking is confirmed once your signed Client Acknowledgement & Agreement is received.
Receive Your Welcome Package
You will receive confirmation of your request, along with the Client Acknowledgement & Agreement and Intake Form.
Complete Your Forms
Sign the agreement and complete the Intake Form. Once received, you will be sent instructions for accessing The Crossing Room.
